Cell Phone Request :
City employees eligible for a city cell phone can request 1) a City-issued cell phone or 2) a Monthly Cell Phone Stipend.  Eligibility for a City phone will be based on your role with the city. City Manager Approval is needed.

Personnel Action Form:
Human Resources Use Only: This form will be used for all City employees who have changed their salary, title, or employment status. The PAF is not complete until the City Manager signs off.

 Benefits Enrollment Packet:

Use this form to enroll in your annual benefits, including medical, dental, and vision, and add your dependents if needed and beneficiaries. The form is only used for new hires, during Open Enrollment, and qualifying Life Events.

Fuel Purchasing Policy

Employees will have access to a fuel card to purchase fuel for authorized City vehicles or City equipment only. Please submit all fuel transactions with receipts to the Finance department.

Handbook Receipt and Acknowledgement:

The employee recognizes, acknowledges, and agrees that they received a hard copy or an electronic copy of the Employee Handbook.

Payroll Direct Deposit Enrollment:

All City employees are required to enroll in direct deposit. Employees must include their financial institution’s name, routing, and account number. Documentation must be uploaded (a copy of the document with routing and account number(s) or a voided check).
